ONLINE REGISTRATION PROCESS

If you will be living within our practice area for more than 3 months, you should register permanently at the Practice.

Registration is with the practice and not an individual GP although you can request an appointment with your preferred GP subject to their availability.

Registration Forms

You must complete both  forms below (GP registration form and Online Medical Questionnaire for New Patients).

GP Registration form  - this form does not need to be downloaded and signed.  You can simply complete, save to desktop, attach to email to pittenweem.surgery2@nhs.scot

PreRegistrationOnline Medical Questionnaire For New Patients (not required for patients under 16 year)

Identification

You should also provide 2 forms of identification

  • Photographic ID (e.g. driving license, passport, travel pass)
  • Proof of address (e.g. utility bill, tenancy agreement, any other official form of correspondence with your new address)

You can scan or take photos of your ID to submit with your application.  Once we have seen these we will delete, we do not retain copies. 

Note that by sending these items you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

What Happens Next?

Once we have received both forms and both items of ID we will complete your registration. We will not routinely contact you to advise you that his has been done but, if you don't hear from us, you can assume that you are now registered with the Practice. 

We make every effort to process applications promptly to ensure a smooth transition from your previous practice.  Please help us by ensuring you complete the forms fully, provide the necessary identification, and ensure that you have enough medication from your previous practice to last for at least 2 weeks.
